1. To enter: Sweepstake begins on May 1, 2012 and ends June 30, 2012 in participating BCD Tofu House restaurants. Each diner will receive an entry form during the sweepstake period or you may ask waitperson for an entry form if one is not offered. Entrants must submit a completed survey: to be “complete” your entry must contain your name, address, phone number and valid email address. Failure to provide complete information will disqualify you from the sweepstakes.
2. Eligibility: This sweepstake is open to individual who are 18 years of age or older and legally reside in the states of California. Employees and their immediate spouse of BCD Tofu House and its affiliated company (advertising/promotion agencies associated with this sweepstakes) are ineligible.
3. Prizes: The 1st prize is one airline ticket with an Approximate Retail Value (“ARV”) of $1,500 in which BCD purchased through A Ju Tour Inc. (3053 W. Olympic Blvd. #101 Los Angeles, Ca 90006). The 1st Prize winner may exchange this airline ticket with any travel package offered by A Ju Tour Inc. within one year from the date of the award. An exchange of the airline ticket with any travel package has to be the same value or less than $1,500. Should a difference arise, the difference will not be awarded in cash or any other prize. The prize winner is responsible for any additional cost if the desired travel package exceeds “ARV” of $1,500. The prize winner is also responsible for any federal, state and local taxes. One (1) 1st prize will be awarded. Two (2) 2nd prizes will be awarded. The 2nd Prize is the new Apple IPAD, 16GB. (“ARV” of $499) Fifty (50) 3rd prizes will be awarded. The 3rd prize is a BCD Tofu House gift certificate (“ARV” value $20) good for one (1) year from the date of award. Odds: Odds of winning the prizes will depend on the number of eligible entries received during the period.
4. Awarding and claim of prize: Each prize winner will be selected via random drawing on July 15, 2012. The winner will be notified within (5) business days after drawing via telephone, email or by USPS mail. If prize notification with affidavit release are unclaimed, or returned as undeliverable, the prize will be forfeited. If any prize winner does not claim their prize, on or before August 15, 2012, the prize will be forfeited and a selection will be conducted from all entries received to award the unclaimed prize. Winner Announcement: To collect prize, winner must sign consent form granting permission for BCD Tofu House to use their name and pictures from sweepstakes and award presentation in its advertisement and/or press release. Prize winner will receive a 1099 IRS Tax form for the value of the prize.
5. General: By participating in this sweepstakes, each entrant agrees to be bound by these official rules and release BCD Tofu House from any liability arising from your participation or the use of the prize. BCD Tofu House is not responsible for illegible, damaged, late or lost entries or failed computer transmissions or technical failures. If technical difficulties compromise the integrity of the sweepstakes, BCD Tofu House reserves the right to terminate the sweepstakes and conduct a selection to award prizes using all entry forms received as of the termination date.
